Name Changes

Students
A request to change your name in your academic record can be made in person or through the mail. For more information, please visit https://www.nova.edu/registrar/services/name-change.html.

Employees
If you are an employee (faculty, staff, temporary and student employees) Federal regulations require employers to validate that the employee's personal information exactly matches the appropriate legal document to ensure proper tax reporting. Thus, in order to reflect your name correctly on your personnel and payroll records, you must provide a copy of your social security card to the Office of Human Resources.

If you have not requested a change of name with the Social Security Administration, please apply for a new Social Security card with the nearest Social Security Administration office. (Note: You will need to bring an original legal document reflecting your new name to support your request for the name change. Examples of accepted legal documents include a birth certificate, marriage license, divorce degree, etc.) Once you receive your new Social Security card, please bring it to the appropriate office. If you have any questions or concerns, please call the appropriate office.

To make a change to your Social Security number, submit a completed Data Change Form along with a copy of your signed Social Security card by email to: studentupdates@nova.edu, by fax to (954) 262-2915, or by mail to:

Employees

Thus, in order to reflect your date of birth and legal sex correctly on your personnel and payroll records, you must provide a copy of your birth certificate to the Office of Human Resources.

Marital Status

Employees
If you are an employee (faculty, staff, temporary and student employees), Federal regulations require employers to validate that the employee's personal information exactly matches the appropriate legal document to ensure proper tax reporting. Thus, in order to reflect your date of birth and legal sex correctly on your personnel and payroll records, you must provide a copy of your marriage license or divorce decree to the Office of Human Resources.

Email Addresses Changes

Students
The university's official means of communications with students is by emails directed to NSU students' @mysnu.nova.edu email accounts. If you need assistance with accessing your NSU email account or forwarding your NSU email, please contact the NSU Help Desk at help@nova.edu or (954)262-4357.

Employees
To ensure timely and accurate delivery of information, all official email communications directed to NSU employees (faculty, staff, temporary and student employees) will be sent exclusively to their NSU-assigned email accounts (addresses ending with @nova.edu.).

NSU also requires a personal e-mail on file for employees. Visit Personal Information and update your Web Application E-Mail Address.

Phone Number Changes

All
Your campus emergency alert number is the number marked as primary in Personal Information. It is the best number at which you can be contacted. If you have additional phone numbers, please add them to the phone number section of the form.

Employees
NSU also requires a personal phone number on file for employees. Visit Personal Information to add/update your Employee phone number.

Address Changes

Students
To update your address, click here.

Also, if you have already applied for your degree conferral/diploma, you must email your address change to the Coordinator of Degree Conferrals and Diplomas as well to make sure your diploma is sent to the correct address.

Employees
All employee related mail including W2's and benefit Information will be sent to your HR/Payroll Address. To update your HR/Payroll Address please click here.
